Eligibility
Unless otherwise specified, the following services are provided for frail aged and younger people with disabilities and their carers living in the Council area. However, individual assessments are conducted to ensure eligibility

Services
  • Centre Based Day Care - Aged. Range of social activities at various locations and days of the week. Transport provided if necessary
  • Cleaning - short-term cleaning with number of visits based on assessed needs.Spring clean once annually. Subsidy applies; costs negotiated at time of assessment
  • Home maintenance - minor tasks only. For larger jobs, contractors utilised. Fees discussed at assessment. Client pays for costs of materials. Tel. 8405 6804 for more information
  • Home modification - limited to simple health and safety requirements. Council will arrange a visit from an Occupational Therapist paying $15 for visit. If grab rails ae installed, client pays only the cost the rails. If a bannister is installed client pays cost of materials with council providing a subsidy of $75.
  • Gardening - One off (once a year)garden tidying up for safety and security purposes; tree pruning. $10 per hour (max 3 hours)
  • Friendly Visiting - Small group of volunteers available for friendly visiting. No fee
  • Shopping - Short-term assistance normally 12 visits annually for frail aged and 6 for younger disabled It may be provided on a fortnighly basis to pay bill, banking and purchase groceries. Contribution advised at assessment
  • Transport - Home Support Van provides door-to-door transport to local shopping centres. Separate Community bus service provides transport on fixed routes throughout the Council for the general community. Gold coin donation. Ph 8405 6702
  • Transport - Personal Medical car transport to doctors, hospitals and specialists appointments (one way only). A mobility assessment is required. Ph 8405 6913 for further information.

  • Outings - Weekend trips held regularly to locations within and outside the Council area. Fees discussed at assessment
  • Safety/Security Assessments. Free home security assessment can be conducted. If client accepts recommnendations, work may be done under Council' Home Modification program
  • Supported Recreation - Social support programs.Various activities at Council venues, transport provided,referrals can be made by doctors, community agencies, friends, family or self, an assessment is required which will be undertaken in people's homes by Social Support staff. Small fee for some activities
