Services

Commonwealth home support program services include:- domestic assistance, shopping, appointment support, transport, medication checks, personal care, individual and group social support, basic gardening and maintenance.
Home care packages Level 1, 2, 3 and 4 - Personalised assistance available to support older people to live at home as they choose. May involve a range of services tailored to individual needs. Service can be offered over 7 days with multiple support visits available depending on clients choices and package.
Physiotherapy and occupational therapy services.
Support groups:- For people living with Parkinson's disease Ascot Club and Men's group Connections dementia specific group including recreational activities. Transport available. resources.
Transition care program - Short term program for older people who have been in hospital and are expected to benefit from rehabilitation and support
