- Health and well being services from professional nursing, social worker, physiotherapy, exercise physiologist, fitness leaders, occupational therapy and podiatry , tel. 1300 224 477
- Services also include chronic condition self management, health promotion, physical fitness, lifestyle planning, counselling, learning opportunities, information, social support and linking with community activities, transition care, falls prevention and Strength for Life program
- Help at Home South - individualised support for older people living at home including subsidised service through the Commonwealth Home Support and Home Care package programs. Includes assistance that supports independence with domestic, personal care, shopping and meals
- Assist with transport and home maintenance
- Social Links - connects people to interest based community activities and opportunities for learning and personal development
- Dementia Services - tailored support services for individual's living with memory loss and their carers
- Carer Respite Options for individuals at home or in the community, including activity based small groups
- Riverview day respite - respite for carers of people with high care needs at Perry Park
