Eligibility:
Residents of City of Prospect/Town of Walkerville/City of Burnside/City of Campbelltown/City of Norwood Payneham St Peters/City of Unley, over 65 years, registered with My Aged Care and holding appropriate CHSP referrals. If you do not fit this criteria and are over 65years, please contact the team to discuss your interest.
