Eligibility:
People with a life limiting illness, their carers and family living in the Central Adelaide Local Health Network region
Access:
Wheelchair access, Accessible toilets, Accessible parking
Aims to enhance quality of life of people with a life limiting illness, their families and carers. Services are provided at Queen Elizabeth Hospital, Royal Adelaide Hospital, Mary Potter Hospice, the patient's place of residence (including residential aged care facilities). The service is provided by a multidisciplinary team including nurses, doctors and social workers.

- Consultation liaison services, medical outpatient clinics, coordination of other services in conjunction with the general practitioner
