Eligibility:
People living in the inner southern metro region who are:
65 years or older
50 years or older of Aboriginal or Torres Strait Islander heritage
Under 65 years or under 50 years of Aboriginal or Torres Strait Islander heritage living with disability
Transport disadvantaged; carers of the people above.
Fees:
Fees based on mileage
The CPN aims is to support independence by assisting people with transport needs.
providing a coordinated transport link for residents of the cities of Marion, Holdfast Bay and Mitcham, to enable community participation, health and wellbeing.

Eligibility for new clients requiring transport will be determined by an assessment conducted by contacting:

My Aged Care (call 1800 200 422) - for people aged 65 years and over, and 50 years and over of Aboriginal or Torres Strait Islander heritage.

Inner Southern Metro CPN (call 8375 6769) - for people under 65 years or under 50 years of Aboriginal or Torres Strait Islander heritage living with disability and those transport disadvantaged
